Frank Burgay Blake died in Christ on Friday morning, October 24, 2003 at Theda Clark Medical Center. He was born February 10, 1924 in New York City, the son of Francis Gilman Blake, M.D., and Marguerite Burgay Blake, M.D. Frank married Filomena Yolanda Ciaccio on December 15, 1962. He was a graduate of Elizabeth Irwin High School in New York City. He held bachelor's degrees from the University of Minnesota and St Louis University and a master's degree in Human Physiology from New York University. He also received a diploma from the School of Industrial and Labor Relations at Cornell University, Ithaca, N.Y. and a diploma from the Air University in Hospital Administration, Montgomery, AL.
He had been responsible for the analysis and planning of many hospital information systems pertaining to the acquisition, preparation, analysis and use of reference systems of clinical material and source data for medical research and education. He also provided technical advice on record keeping devices in light of changing diagnostic techniques and had assisted medical staffs in research, administration and professional training programs, and in the preparation of material for publication in professional journals. Before accepting a position in Wisconsin, he was Assistant Administrator for the Continental Air Command, United States Air Force and received an incentive award along with a sustained Superior Performance Rating from the Surgeon General's Office, Headquarters, USAF, Washington, D.C. by maintaining a superior high quality of professional training programs for the Air Force medical staff. As Liaison Administrator for Saint Francis Hospital, Bronx, N.Y., he established education and training programs as necessary for orientation in adjudication requirements. He was a representative for the Medical Case History Bureau, New York City and lectured to Pathologists and Cardiologists on the installation of Cancer Registry and Heart Registry Systems at Montefiore Hospital, Bronx, N.Y.; Babies Hospital, New York City; Bryn Mawr Hospital, Bryn Mawr, Pennsylvania and Presbyterian Hospital, Newark, New Jersey. He was Assistant Department Director at Kings County Hospital Center, Brooklyn, N.Y. Metropolitan Hospital, New Your City; Veterans Administration Hospital, New York City. Department Director at Rahway Hospital, New Jersey; United States Army Hospital, Fort Ord, CA and Tulare County General Hospital, Tulare, CA. He was a Department Head at Winnebago Mental Health Institute and worked on many research projects with the medical staff. The sophisticated research projects he completed with the medical staff were: Surgical Experience at Winnebago 1970-71, Phenothiazine Study 1972 (Published in the Journal of Autism and Childhood Schizophrenia), Profile of Geriatric Discharges to the Community 1973, Profile of Patients that were Cross-Matched with a Diagnosis of Schizophrenia 1968-72, Suicide and Suicidal Attempts of Winnebago Patients, 1972. Profile of Activity of Young Adult Unit with Treatment Modalities 1972-73, Huntington's Chorea 1963-72, Profile of Population Movement 1971-72, and Profile of Activity and Treatment on the Children-Adolescent and Children's Consultation Services 1968-72. Mr. Blake was a Consultant for Bethel Home, Oshkosh; Care Center, Incorporated, Fond du Lac; Brown County Mental Health Center, Green Bay; Fond du Lac Mental Health Center, Fond du Lac, Omro Care Center, Omro and Lake Winnebago Area-Wide Comprehensive Health Planning Council, Oshkosh. He was a Clinical Instructor, Direct Practice, Medical Record Technician Program, Moraine Park Technical Institute, Fond du Lac and Preceptor University of Minnesota Independent Study Program in Mental Health Administration.
Mr Blake transferred from Winnebago Mental Health Institute to the Bureau of Correctional Health Services, Madison. While with the Bureau, he developed an instruction manual for the problem oriented medical record for the correctional institutions in the state. A copy of this manual can be found in the State Historical Society and the Legislative Reference Bureau. He is listed in the international Men of Achievement and Marquis Who's Who in the Midwest.
Mr. Blake was a member of the International Platform Association and Vice-President and President of AARP Chapter 450, Winnebago County. He was on the board of the coalition of Wisconsin Aging Groups from District 3 and later became the State Secretary for the Coalition. He also was a member of the board of the Oshkosh Committee on Aging and a member of the Candlelight Club. Mr. Blake was a member of AARP Wisconsin State Legislative Committee, AARP Work Force Consultant, and AARP Health Care America Speaker. He was invited by AARP to attend meetings in Washington, D.C. and San Diego, CA.
Frank is survived by his wife of 40 years, Filomena of Oshkosh; and sons Anthony Blake of Oshkosh and Robert Blake of Oshkosh. A memorial service will be held at a later date.
